PatientClick converts your current practice forms to a digital format so they are accessible to your patients to fill-out as online forms. Patients can go to the patient portal and fill out all the necessary forms and information before meeting with a provider.

Existing paper forms are scanned in, or recreated in a digital format so that patients and providers can fill out necessary information and apply digital or written signatures, as well as other options.

Once forms are completed they can automatically be stored in the patients electronic health record.
